Okay, I'm not sure if that's a book with one author, but my answer is for a book with one author:
1. Author's name (last name, first name)
2. Title of the book
3. City where it was published
4. Publisher, (leave out words such as "company" and "Inc.")
5. Copyright year
(Example: Anderson, Amy. How To Cite Right. San Diego: Bridge Publications, 2005.)
