While I don't know the exact choices, if any, the most susceptible answer would be because the patient is going through a lot of stress, and on top of the patients unhealthy habits, is unable to handle the stress in a positive, or healthy way, so the patients body is reacting in the way it is. (This is my first answer on BRAINLY so please don't be upset.)
