Using the Gibb's reflective tool, write a reflective report on the two (2) different professional interactions you had from Assessment task two (2). Include in your report how your interaction contributed to the health outcomes of the patient/patients involved, including how you applied principles of wellness to your nursing practice, in your care provided. You must also identify one (1) primary health and one (1) tertiary health care agency/service that you could recommend for the patient/s to assist in improving their health outcomes.
